Once you've scanned a document and verified the scan is legible and the file saved correctly - rip the paper in half. That way, even if it ends up on the floor, or mindlessly put down somewhere before being disposed of, or falls out of the container being used for trash, you know it's already scanned when you come across it again. This has saved me a lot of irritation.
